Fibre Channel (FC) and Fibre Channel over Ethernet (FCoE) driver lpfc might occasionally display a purple diagnostic screen on VMware ESXi 5.5 and 6.0. The diagnostic output includes #PF Exception 14 in world, addr 0x0, and lpfc_scsi_cmd_iocb_cmpl.

This could happen when the driver receives an SCSI Check Condition response and perceives the SCSI Sense Data is either zero length, or is invalid in the FCP_RSP field.
This issue only affects the 11.1 lpfc driver series prior to 11.1.183.633. The 10.x lpfc driver series is not affected by this issue.
For VMware ESXi 6.5, refer to Customer Advisory a00022166en_us, Host Bus Adapters - Fibre Channel Driver lpfc 11.1 Might Encounter a Purple Diagnostic Screen Displaying Page Fault #PF Exception 14 on VMware ESXi 6.5, which will be published in early September 2017.
Upgrade to driver version 11.1.183.633 or newer. Install the latest firmware that matches the 11.1 series driver. Upgrading to driver version 11.1.183.633 does not include a specific matching firmware update.
For adapters that are supported on the new 11.2 series lpfc driver, upgrade to the 11.2.x.x series lpfc driver. Be sure to also install the matching 11.2 firmware.
Highly recommended: Upgrade Server System ROM and iLO to April 2017 Service Pack for ProLiant (SPP) release or newer to assure compatibility. Mixing new adapter driver and firmware with older server System ROM and iLO has not been qualified.
At the time of this writing, the following adapters are not supported by the 11.2 lpfc driver and must use the 11.1 series lpfc driver (11.1.183.633 or newer) for VMware ESXi 5.5 and 6.0:

- HPE FlexFabric 10Gb 2-port 554M Adapter

- LPe1205 8Gb Fibre Channel Host Bus Adapter for c-Class BladeSystem
