IMPORTANT: A potential mild electrical shock that, if experienced, may cause a Startle Reaction (a defensive, involuntary response) may occur with HP 800W Gold (Redundant) / 900W (Non-Redundant) AC Power Input Modules. The shock is described as a mild tingling sensation and personal injury is unlikely to be caused by the shock itself. The power module replacement described in the Resolution is considered a critical fix and is required to correct the issue detailed below. HP strongly recommends immediate application of this critical fix. Neglecting to perform the required action could result in a server configured with a power module presenting a potential mild electrical shock hazard. By disregarding this notification and not performing the recommended resolution, the customer risks incurring an electrical shock in the future.

HP has identified HP 800W Gold (Redundant) / 900W (Non-Redundant) AC Power Input Modules (Part Number 745710-201, Spares Part Number 754376-001) that have a potential to cause a mild electrical shock sensation if contact is made with the AC inlet or an AC power cord connected to the module but unplugged from the AC supply when two power modules are installed in a system. The electrical shock is perceptible and described as a mild tingling sensation. However, if experienced, the shock may result in a Startle Reaction (a defensive, involuntary response). As a result, HP has developed the proactive replacement program defined in the Resolution section below.
Note: HP 800W Gold (Redundant) / 900W (Non-Redundant) AC Power Input Modules with Part Number 745710-202 and Spares Part Number 754376-001 are not affected by this issue.
SCOPE
Any of the following HP ProLiant Gen9 servers configured with an HP 800W Gold (Redundant) / 900W (Non-Redundant) AC Power Input Modules (Part Number 745710-201, Spares Part Number 754376-001) with a CT/Serial Number that has "DXSH" in digits 2 - 5 as shown in Figure 1 (see Resolution):
- HP ProLiant DL120 Gen9
- HP ProLiant DL160 Gen9
- HP ProLiant DL180 Gen9
- HP ProLiant DL60 Gen9
- HP ProLiant DL80 Gen9
- HP ProLiant ML150 Gen9
RESOLUTION
Gather the CT Numbers for HP 800W Gold (Redundant) / 900W (Non-Redundant) AC Power Input Modules that must be remediated and click the following URL to enter power module CT/Serial Numbers for validation and to order replacement power modules through the OneHP website:
Note: Do not order replacement power modules via the standard support process.
Affected power modules must be returned to OneHP within 30 days of receiving replacement power modules. Please use the packaging that came with the replacement power modules to return your modules to OneHP. If OneHP does not receive affected power modules within 30 days, the customer (or entity who ordered the replacement power modules) will receive a bill for any unreturned power modules.
Note: A list of CT/Serial Numbers for potentially affected power modules at the customer site will be required. Proactive remediation of power modules that are not eligible for replacement is neither necessary nor authorized.
IMPORTANT: Affected power modules should be returned to HP within 30 days of receiving replacement power modules.
CT/Serial Numbers: HP 800W Gold (Redundant) / 900W (Non-Redundant) AC Power Input Module CT Numbers can be found on the power module's product label or in HP Integrated Lights-Out (iLO) under the "Serial" heading by navigating from System > System Information > Power Supply Summary.
Figure 1: HP 800W Gold (Redundant)/900W (Non-Redundant) AC Power Input Module CT Number listed on the product label.
Figure 2: HP 800W Gold (Redundant) / 900W (Non-Redundant) AC Power Input Module CT/Serial Numbers are listed under the "Serial" heading by navigating to System > System Information > Power Supply Summary in iLO. (Please note that affected power modules have Spares Number 754376-001 and "DXSH" in digits 2 - 5 of the CT/Serial Number.)
IMPORTANT: Potential Mild Electrical Shock. Exercise caution when replacing HP 800W Gold (Redundant) / 900W (Non-Redundant) AC Power Input Modules. When two HP 800W Gold (Redundant) / 900W (Non-Redundant) AC Power Input Modules are installed in a system and the AC power cord is removed from one power module, electrical current will continue to be present on the exposed connector pins of the C-14 Power Input Connector (Item 4 in Figure 3 below) or the pins of the power cord if left plugged into the module due to current leakage from the active power module. Affected Power Input Modules were manufactured prior to 26 March 2015 and are labeled with Part Number 745710-201, Spares Part Number 754376-001, and "DXSH" in digits 2 - 5 of the CT/Serial Number.
